I'd rather tell you: It was FormMail.pl from Matt's Script Archive.
(Right now it's disabled for obvious reasons.)

FormMail.pl can be made relatively secure by altering the 'allowed referers'
See http://www.worldwidemart.com/scripts/readme/formmail.shtml and look at
the section under 'setting up the script"
I don;t know how you had formmail setup but it defaults to allowing anyone
to utilise the script. You need to 'lock down' the allowed referers to only
allow the specified hosts to initiate mail sending.
